SERVICE INFORMATION
Identification Cards.
Each member receives a Kaiser Permanente identification card with a unique identification number. Please remember to bring your card with you when you visit or call the Medical Offices and carry it with you at all times.
Arranging Your Care.
Medical Group Physicians, nurses and other providers will all participate in the delivery of your health care. When you first call your Medical Office, you may ask the New Member Health Coordinator (a registered nurse) to assist you in selecting Physicians for your family members. Each member should choose a primary care physician practicing at that Medical Office. This Medical Group Physician will provide or arrange for all necessary medical care.
Hospital Care.
Your Medical Group Physician will discuss the need for your hospitalization when appropriate and make the necessary arrangements prior to your admission. The hospitals that we contract with are Saint Joseph Hospital, The Children's Hospital and Boulder Community Hospital.
Second Opinions.
You may obtain a second opinion, regarding proposed surgery or other treatment, from any Medical Group Physician subject to payment of any applicable Supplemental Charges. Your primary Medical Group Physician or the Member Representative at your Medical Office can help you select an appropriate physician/specialist. A referral will be made ONLY if we are unable to provide an appropriate second opinion in-Plan. If you elect to obtain a second opinion from a non-Medical Group Physician without a referral, you must pay for such services yourself.
