1. Family Medical Leave Act (F.M.L.A.) District pays health insurance for up to 12 weeks. Must meet guidelines and provide medical validation. For parent, spouse, child, self. Includes pregnancy, adoption, foster child. Information and forms are available in Employee Health Services.

2. Personal Accumulated Sick Days - Eligible employees may use all accumulated sick days with full pay and full Benefits.
Return to work or if unable

3. Sick Leave Bank Days - Only employees who have joined this program can participate. Members receive full pay and full Benefits.

Return to work or if unable

4. Personal Illness LeavelFamily Illness &emdash; DPS provides no pay or benefits.No pay; no benefits are provided by DPS.

Leave period - one year: can be extended to two years.

Information and forms are available in Employee Health Services.

Any benefits must be paid by employee.

5. Restoration of Health Leave

Must have completed at least ten (10) years of active service

1/2 pay, health and life insurance; if you wish to have this year count toward
retirement, you pay in your portion. Contact Retirement Office 303-813-4653

6. Long Term Disability (LTD) - Income Insurance - Metropolitan or UNUM
(Employee Benefits)

Full time employees are eligible upon completion of three months of continuous
service. Please call Flora Osenda 303-764-3374 for additional information. You must be off work 6 months before payments (if approved) Return to work or if unable

7. Regular Retirement

8. Disability Retirement

a. Meet with Dr. Alan Belstock 303-813-4651. If you apply then;

b. Dr. Patricia Motz meets with you to obtain History and Current Medical Records.

c. Letter from your Dr. stating you are "permanently incapacitated from current job functions." Also, need medical records from your physician(s).

d. Physical exam scheduled with Dr. Kathryn Mueller; may also be required to see a psychiatrist if emotional/mental condition is present.

e. Decision &emdash; approval/denial.

f. If approval is granted, a yearly review is required.

g. If denied, options will be discussed.
